COMPREHENSIVE RELEASE, MEDICAL RELEASE OF LIABILITY, AND RELEASE TO OBTAIN MEDICAL CARE
I am participating in a trip that has been arranged by Mission of Hope (MOH) to take place on one or more MOH campuses in the Caribbean or Central America. As a condition of participation in this trip, I understand that I must execute this Comprehensive Release, Medical Release of Liability, and Release to Obtain Medical Care (herein “Release”) in favor of MOH. This trip may be to provide voluntary services or as part of my employment, to participate in a vision trip, or to provide services, which may include professional services, in aide of MOH’s missions and activities (herein “Mission Trip”). I understand that MOH is a nonprofit organization that relies upon volunteers to conduct its missions and activities and, accordingly, I recognize I am assuming the risks of travelling to and from and participating in a MOH Mission Trip to one or more foreign countries. I will receive material or immaterial personal benefits and personal satisfaction, which I consider to be good and sufficient consideration for execution of this Release. I hereby release and discharge Mission of Hope, Inc., its members, directors, members of Board of Council, C-Class officers, officers, agents, and employees and others who provide services to MOH, either for payment or on a volunteer basis, and all subsidiary and partnering organizations, controlled by MOH , as well as local host ministries and organizations (all together, “MOH” or “Mission of Hope”) from any and all claims, now or during the Mission Trip, or arising in the future after the Mission Trip, for any and all forms of personal injuries or property damage that I or my family may suffer as a result of my participation in the Mission Trip, whether or not such injuries or damage are caused by any action, inaction, negligence (active or passive) by any of the entities or individuals mentioned above, or any circumstance, hazard, natural hazard, event, or occurrence I may encounter during, after, or as a result of, my participation in a MOH Mission Trip.
I am aware that the United States Department of State (US State Department) and the foreign equivalent departments issue various travel advisories, warnings, suggestions, and advice regarding domestic and international travel. I will familiarize myself with all current advisories, warnings, suggestions, and advice which may pertain to my Mission Trip and am responsible to remain informed of developments or changes in such advisories, warning, suggestions, and advice throughout the duration of my Mission Trip. I understand MOH is unable to provide to me any form of insurance, or traveler’s health insurance during my Mission Trip. I acknowledge that MOH has advised me to seek and secure any other and additional forms of insurance that may be available to me before participating in the Mission Trip. I acknowledge that MOH has advised me to seek any other professional or legal advice before signing this Release. I hereby knowingly and voluntarily assume all responsibility for any and all risks of property damage or bodily injury that I may sustain while participating in, or arising later as a result of my past participation any voluntary relief effort, disaster exercise, or other activity of any nature, including the use of vehicles, boats, airplanes, equipment, and facilities of MOH, or volunteers on this Mission Trip.
If I should need medical care during or as a result of the Mission Trip, for any reason, I agree to assume any and all responsibility for any and all direct or indirect costs incurred for medical treatment, medications, supplies, and medical transportation while on this Mission Trip. I agree to assume any and all responsibility for any and all direct or indirect costs which may arise after the Mission Trip. I agree to assume any and all responsibility for any and all direct or indirect costs and liabilities which may arise during transportation to, from, and around the site of my Mission Trip.
